Enhance the quality of patient care as a Nurse Practitioner specializing in home health assessments for Medicare Advantage members in Knox County. This per diem opportunity offers flexibility and autonomy, allowing you to create your own schedule with shifts available any day of the week. Coverage areas include communities such as Farragut, Powell, and Galesburg, where youll meet patients in the comfort of their homes and provide critical risk assessments.
Schedule:
- Fully flexible shifts, Monday through Sunday
- Self-scheduling to suit your availability
- Minimum hourly commitments on a weekly or bi-weekly basis
Core Responsibilities:
- Conduct annual home health risk assessments, averaging 45 minutes per visit
- Review comprehensive medical history, current conditions, and medication profiles
- Deliver education to patients about their medical conditions and prescribed medications
- Identify care gaps and offer actionable recommendations for optimized health outcomes
- No requirements for treatment planning or medication prescribing
Preferred Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification (AANP or ANCC)
- MSN degree from an accredited school of nursing
- Current BLS/CPR certification
- Six months or more of experience as a practicing NP is preferred
