Enjoy a dynamic, community-centered clinical role that offers the autonomy and flexibility of per diem work. As a Nurse Practitioner specializing in Home Health, you will conduct comprehensive risk assessments for Medicare Advantage members throughout Henry County, IN, including New Castle, Middletown, and Knightstown. These patient-centered visits, averaging 45 minutes each, may include annual wellness assessments or post-discharge evaluations, supporting overall patient well-being in their home environment.
Schedule/Hours:
- Flexible shifts available Monday through Sunday
- Option to create your own schedule
- Commitment may be scheduled weekly or bi-weekly
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage participants
- Review and assess medical history, current medical status, and medication usage
- Deliver education to patients regarding diagnoses and medication management
- Identify care gaps and recommend follow-up actions to ensure optimal health outcomes
- No need for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Certification in Basic Life Support (CPR/BLS)
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Minimum of 6 months current NP experience preferred
This per diem opportunity offers an attractive per-evaluation rate of $80-$90, in addition to a mileage stipend and compensation for administrative duties.
