Take on a vital role in supporting community health as an experienced Nurse Practitioner for home health risk assessments. This is a flexible per diem opportunity in Dare County, NC, including areas such as Kill Devil Hills, Atlantic township, Nags Head township, and Hatteras township. You will have the autonomy to create your own schedule, choosing shifts between Monday and Sunday, and can commit to a minimum of weekly or bi-weekly hours that suit your availability.

Core Responsibilities:
- Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients prior and current medical history, existing conditions, and medication history
- Provide education and guidance regarding health conditions and proper medication use
- Identify gaps in care and offer actionable recommendations to improve patient outcomes
- No need to develop formal treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current BLS (CPR) certification
- Board Certification as a Nurse Practitioner (AANP or ANCC)
- 6 months or more recent experience as an NP preferred
- Masters degree in Nursing (MSN) from an accredited institution
