Advance your career by performing impactful work in home health care as a Nurse Practitioner, delivering in-person risk assessments to Medicare Advantage members across Dare County. This per diem opportunity offers flexibility and autonomy while supporting the wellbeing of patients in their own homes, including those in Atlantic, Nags Head, Hatteras, and adjacent communities.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own work schedule
- Choose your level of weekly or bi-weekly commitment
Key Responsibilities:
- Complete comprehensive home health risk assessments (approx. 45 minutes per visit) for Medicare Advantage plan members
- Review medical history, assess current conditions, and evaluate medication regimens in a home setting
- Educate patients and families regarding ongoing health concerns and medication management
- Identify gaps in care and recommend follow-up actions as needed
- Focus solely on evaluationsno prescribing or treatment planning involved
Qualifications:
- Valid Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC required
- MSN degree from an accredited program
- Current BLS/CPR certification
- Minimum 6 months of recent NP experience preferred
