Job Description
Deliver in-home care and make a positive impact by conducting home health risk assessments for Medicare Advantage members throughout Osceola County, FL. As a Nurse Practitioner specializing in home health, you will perform comprehensive assessments in Kissimmee, St. Cloud, Poinciana, and nearby areas. This per diem opportunity offers exceptional flexibility, empowering you to create your own schedule while contributing valuable insights to promote patient well-being.
Schedule & Hours
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment required
Primary Duties
- Perform annual risk assessments for Medicare Advantage health plan members during home visits (approximately 45-minute sessions)
- Review and evaluate patients medical histories, current conditions, and medication usage
- Educate patients on ongoing medical issues and medication management
- Identify gaps in care and provide actionable recommendations for improved outcomes
- Focus exclusively on assessment, education, and recommendationsno need to prescribe medications or create formal treatment plans
Qualifications & Experience
- Active Nurse Practitioner license in Florida
- Current CPR/BLS certification
- Board certification through AANP or ANCC
- MSN from an accredited school of nursing
- At least 6 months of recent NP experience preferred
- Strong communication and assessment skills
