Job Description

Advance your career as a Nurse Practitioner specializing in home health by conducting in-depth risk assessments throughout Jasper County, including the communities of Kirbyville, Buna, and Sam Rayburn. Focused on supporting Medicare Advantage plan members in their own homes, this per diem role offers unmatched flexibility and autonomy.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule according to your availability
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform home-based annual health and post-discharge risk assessments for Medicare Advantage members
  • Evaluate comprehensive medication, medical history, and current conditions during each assessment (approximately 45 minutes per visit)
  • Educate patients and their caregivers about managed health conditions and medications
  • Identify care gaps; recommend actionable steps for resolution
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Texas
  • Current BLS (CPR certification)
  • Board Certified through AANP or ANCC
  • MSN from an accredited nursing program
  • At least 6 months of recent Nurse Practitioner experience strongly preferred
