Job Description

Opportunity for a dedicated Nurse Practitioner with experience in home health to conduct risk assessments for Medicare Advantage members across Pittsburg County, OK. This per diem position offers outstanding flexibility, allowing you to create your own schedule while making a meaningful impact in the lives of patients residing in McAlester, Hartshorne, Krebs, Quinton, and other locations within the county.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule to fit your lifestyle
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Perform comprehensive annual health risk assessments in patients homes
  • Review and evaluate patients medical histories, current health conditions, and medication regimens
  • Educate patients on their medical conditions, medications, and preventive care
  • Identify and address any gaps in care, offering recommendations for resolution
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner (NP) License in Oklahoma
  • Current CPR certification (BLS required)
  • Board Certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • Minimum of 6 months of recent NP experience preferred
