A rewarding per diem opportunity is available for a Nurse Practitioner eager to make a direct impact in the home health space. This role involves conducting comprehensive risk assessments in the homes of Medicare Advantage plan members across Monongalia County, WVincluding Morgantown, Brookhaven, and Cheat Lake. Each visit, averaging 45 minutes, may focus on annual assessments or post-discharge evaluations tailored to each patient's needs.

• Flexible shifts available Monday through Sunday
• Self-schedulingchoose shifts that fit your availability
• Set your own minimum commitment weekly or bi-weekly

Key Responsibilities:

• Complete thorough home health risk assessments for Medicare Advantage members
• Review and evaluate medical history, chronic and acute conditions, and medication usage
• Educate patients and caregivers on diagnoses, treatments, and medication regimens
• Identify any care gaps and advise on necessary next steps or follow-up care
• No requirement for ongoing treatment management or medication prescribing

Qualifications:

• Valid Nurse Practitioner license in West Virginia
• Board certification through AANP or ANCC
• Current CPR (BLS) certification
• MSN from an accredited nursing program
• At least six months of Nurse Practitioner experience strongly preferred

This per diem assignment provides an attractive compensation structure, paying $80-$90 for each evaluation, plus a mileage stipend and payment for associated administrative tasks.
