A skilled Nurse Practitioner is needed to perform home health risk assessments for Medicare Advantage health plan members in Scranton and surrounding areas within Lackawanna County, PA. This per diem opportunity allows you to create your own schedule and work flexible shifts between Monday and Sunday, with minimum weekly or bi-weekly hourly commitments to fit your lifestyle.

Schedule:

• Flexible shifts, Monday through Sunday
• Self-scheduling for maximum flexibility
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments during 45-minute home visits
• Evaluate patients prior and current medical history, medical conditions, and medication use
• Educate patients on their medical conditions and medication regimens
• Identify gaps in care and offer clear recommendations to address these issues
• Visits may include annual health assessments or post-discharge follow-ups
• No requirement for creating treatment plans or prescribing medications

Qualifications:

• Valid Nurse Practitioner license in Pennsylvania
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• Minimum 6 months of recent NP experience preferred
• MSN degree from an accredited nursing program
