Advance your career as a Nurse Practitioner specializing in home health risk assessments across Greene County, NY. This per diem position gives you the flexibility to design your own schedule, with shifts available throughout the weekincluding weekends. Assignments are within Catskill, Cozsackie, Ashland, Ravena, and the surrounding areas, ensuring engaging work while providing essential care to Medicare Advantage plan members.
Schedule:
- Flexible shifts Monday to Sunday
- Set your preferred hours to align with your availability
- Commitment can be weekly or bi-weekly
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in their homes
- Review prior and current medical history, assess ongoing medical conditions, and document medication histories
- Provide clear education to patients on their medical conditions and prescribed medications
- Identify gaps in patient care and make appropriate recommendations for resolution
- All assessments are non-clinical; no prescription writing or treatment planning is necessary
Qualifications:
- Active Nurse Practitioner License in New York
- Board Certification through AANP or ANCC required
- Current CPR (BLS) certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least six months of recent Nurse Practitioner experience preferred
