The typical assessment visits last about 45 minutes and may involve annual health evaluations or post-discharge check-ins, all conducted in the comfort of each member's home.

Schedule:

• Flexible shifts available between Monday and Sunday
• Design your own work schedule
• Choose your weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete comprehensive annual home health risk assessments for Medicare Advantage plan members
• Review medical history, current conditions, and medication usage
• Educate patients on their medical conditions and prescribed therapies
• Detect gaps in care and deliver actionable recommendations
• No requirement for treatment plan creation or medication prescribing

Qualifications:

• Active Nurse Practitioner license in Idaho
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC
• Six months of recent experience as a Nurse Practitioner preferred
• MSN degree from an accredited nursing program

This position offers compensation of $80$90 per evaluation, with additional mileage reimbursement and administrative time compensation.
