Job Description
Deliver quality patient care while enjoying autonomy and a highly flexible per diem schedule. This advanced practice role focuses on conducting in-home health risk assessments for Medicare Advantage plan members in various communities throughout Shoshone County. Each assessment is performed in the patients residenceincluding areas such as Kellogg, Pinehurst, and Osburnand typically takes 45 minutes per visit. Assignments may encompass annual wellness reviews as well as post-discharge evaluations, supporting comprehensive patient care directly in the home setting.

- Flexible scheduling available Monday through Sunday
- Self-scheduling and shift creativity encouraged
- Commit to a minimum number of weekly or bi-weekly hours that fit your lifestyle
Responsibilities
- Perform thorough home health risk assessments for designated health plan members
- Review and document detailed medical histories, current health status, and medication regimens
- Educate patients about their health conditions and prescribed medications
- Identify gaps in care and share recommendations for ongoing health management
- No requirement for prescribing medication or developing treatment plans during assessments
Qualifications
- Nurse Practitioner licensure in Idaho
- Current BLS-certified CPR credential
- Board Certified through AANP or ANCC
- At least 6 months of NP experience preferred
- Master of Science in Nursing (MSN) degree from an accredited program
