Job Description
Elevate your nursing career as a Nurse Practitioner specializing in home health risk assessments. In this per diem opportunity, you will leverage your expertise to make impactful assessments for Medicare Advantage plan members residing within Shoshone County, including Kellogg, Pinehurst, and Osburn. Each evaluation, tailored to patient needs, will last around 45 minutes and may range from annual assessments to post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Commit to a minimum number of hours weekly or bi-weekly
Key Responsibilities:
- Conduct comprehensive annual risk assessments in patients homes for Medicare Advantage members
- Review patients prior and current medical histories, conditions, and medication use
- Educate patients regarding their health conditions and prescribed medications
- Detect gaps in care and offer actionable recommendations to address them
- Travel within Shoshone County (Kellogg, Pinehurst, Osburn, and nearby areas) as assignments require
- No need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Idaho
- Board Certification through AANP or ANCC
- Valid and current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least six months of current experience as a Nurse Practitioner preferred
