Elevate your Nurse Practitioner career by providing essential home health risk assessments to Medicare Advantage plan members. This per diem opportunity centers on delivering quality, patient-centered care within Dallas County, ARincluding areas such as Fordyce, Sparkman, and Carthage. As a Nurse Practitioner conducting these home-based assessments, you'll play a pivotal role in empowering patients with knowledge and ensuring they remain on the path to optimal health.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive annual home health risk assessments (approximately 45 minutes per visit)
- Evaluate each patient's medical history, current conditions, and medication regimen
- Educate patients on their health status, chronic conditions, and prescribed medications
- Highlight care gaps and provide actionable recommendations to improve outcomes
- No requirement to create long-term treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Arkansas
- MSN degree from an accredited nursing school
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- At least 6 months of recent Nurse Practitioner experience preferred
