Job Description

A rewarding per diem opportunity is available for an experienced Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members. This role focuses on providing exceptional care and patient education within Lincoln County, NCincluding Iron Station, Lowesville, Westport, and surrounding areas. Each assessment is expected to take approximately 45 minutes and may involve annual health evaluations or post-discharge visits, with travel to different locations as needed.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Choice to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
  • Review and evaluate patients prior and current medical history, existing conditions, and medication use
  • Educate patients about their health conditions and prescribed medications
  • Identify potential gaps in care and recommend practical solutions
  • No requirement for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • Preferred: At least 6 months of recent experience as a Nurse Practitioner
  • Master of Science in Nursing (MSN) degree from an accredited institution
