Job Description

Deliver high-quality patient care as a Nurse Practitioner specializing in home health risk assessments in Jerome County, ID. This per diem opportunity offers the flexibility to design your own schedule, with shifts available any day of the week. Candidates may be assigned to visit Medicare Advantage members residing in various locations within Jerome County, including Jerome, Hazelton, Eden, and surrounding areas. Each assessment typically lasts about 45 minutes and may involve an annual health assessment or post-discharge visit, focused on comprehensive patient evaluation and care recommendations.

Schedule:

  • Flexible shifts available Monday through Sunday.
  • Self-scheduling with minimum weekly or bi-weekly commitments.

Key Responsibilities:

  • Conduct in-home risk assessments for Medicare Advantage plan members.
  • Review and evaluate medical history, current conditions, and medications.
  • Educate patients regarding their medical status and prescribed therapies.
  • Detect care gaps and suggest practical solutions for resolution.
  • Documentation and administrative tasks related to each assessment.
  • No requirement to create treatment plans or prescribe medications.

Qualifications:

  • Active Nurse Practitioner License in Idaho.
  • Master of Science in Nursing (MSN) from an accredited program.
  • Board Certification through AANP or ANCC.
  • CPR certification (BLS) is required.
  • Six months of recent Nurse Practitioner experience preferred.
