Job Description

Embark on a dynamic healthcare role supporting Medicare Advantage members through home health risk assessments within Twin Falls County, ID. Duties and Responsibilities:

  • Perform comprehensive in-home risk assessments (approximately 45 minutes each) for Medicare Advantage members throughout Twin Falls, Jerome, Filer, and other areas within Twin Falls County
  • Review and assess past and current medical history, chronic conditions, and medications
  • Educate patients about their medical conditions, medications, and wellness strategies
  • Identify unaddressed health needs or gaps in care and offer recommendations to improve quality of life
  • No prescribing or treatment planning required; focus is on assessment and education

Required Qualifications:

  • Active Nurse Practitioner License in Idaho (ID)
  • Current CPR certification (BLS)
  • Board Certification as an NP through AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of current nurse practitioner experience strongly preferred
