Embark on a dynamic healthcare role supporting Medicare Advantage members through home health risk assessments within Twin Falls County, ID. Duties and Responsibilities:
- Perform comprehensive in-home risk assessments (approximately 45 minutes each) for Medicare Advantage members throughout Twin Falls, Jerome, Filer, and other areas within Twin Falls County
- Review and assess past and current medical history, chronic conditions, and medications
- Educate patients about their medical conditions, medications, and wellness strategies
- Identify unaddressed health needs or gaps in care and offer recommendations to improve quality of life
- No prescribing or treatment planning required; focus is on assessment and education
Required Qualifications:
- Active Nurse Practitioner License in Idaho (ID)
- Current CPR certification (BLS)
- Board Certification as an NP through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of current nurse practitioner experience strongly preferred
