Job Description
Deliver high-quality care directly to patients homes by performing comprehensive health risk assessments for Medicare Advantage members across Dare County. This per diem role is perfect for Nurse Practitioners looking for schedule flexibility and independence, as youll conduct home visits in communities including Kill Devil Hills, Atlantic township, Nags Head township, and Hatteras township.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own workdays and start times
- Only a minimum weekly or bi-weekly hourly commitment required
Core Responsibilities:
- Complete in-depth annual or post-discharge health risk assessments, averaging 45 minutes per visit
- Review patients medical histories, evaluate current medical conditions, and assess medication usage
- Educate patients on managing medical conditions and medications
- Detect any gaps in care and recommend next steps to improve outcomes
- No prescribing or treatment plan creation necessaryfocus is on assessment and education
Qualifications and Experience:
- Active Nurse Practitioner license in North Carolina
- Board certification through AANP or ANCC
- Current BLS CPR certification
- MSN degree from an accredited nursing program
- At least 6 months of Nurse Practitioner experience is preferred
This opportunity offers competitive compensationearn $80-$90 for each completed evaluation, along with a mileage stipend and administrative time paid.
