Dynamic opportunities are available for Nurse Practitioners passionate about providing in-home care and making a meaningful impact in their community. This per diem role involves conducting comprehensive home health risk assessments for Medicare Advantage plan members residing in Monroe County, FL. The position offers a high degree of flexibility, with the ability to set your schedule and choose your preferred shifts.
Schedule:
- Flexible shifts available any day between Monday and Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Perform annual, post-discharge, and situational home health risk assessments, averaging 45 minutes per patient
- Assess patients prior/current medical conditions, medication history, and overall health status
- Offer education to patients regarding their medical conditions and medications
- Identify gaps in care and make recommendations to support better health outcomes
- No requirements for creating treatment plans or prescribing medication
Qualifications:
- Valid Nurse Practitioner (NP) license in Florida
- Current BLS certification (CPR)
- Board Certification through AANP or ANCC
- At least six months of recent NP experience preferred
- MSN from an accredited nursing school
Additional Information:
- Service areas include Key West, Key Largo, Marathon, and other Monroe County locations; candidates may be scheduled in any of these areas based on shift needs.
- Compensation ranges from $80$90 per evaluation, with additional mileage stipend and administrative time included.
