This per diem position offers the opportunity to work independently while delivering high-quality, patient-centered care across Kernersville and the greater Forsyth County area.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your lifestyle
- Meet a minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
- Assess prior and present medical history, current conditions, and medication use
- Provide education regarding medical conditions and medication adherence
- Detect gaps in care and recommend ways to improve health outcomes
- No need to develop treatment plans or prescribe medicationsfocus on patient evaluation and education
Qualifications:
- Active Nurse Practitioner (NP) license in North Carolina
- Board certified via AANP or ANCC
- Valid CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least six months of current NP experience preferred
