Job Description

This per diem position offers the opportunity to work independently while delivering high-quality, patient-centered care across Kernersville and the greater Forsyth County area.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to fit your lifestyle
  • Meet a minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
  • Assess prior and present medical history, current conditions, and medication use
  • Provide education regarding medical conditions and medication adherence
  • Detect gaps in care and recommend ways to improve health outcomes
  • No need to develop treatment plans or prescribe medicationsfocus on patient evaluation and education

Qualifications:

  • Active Nurse Practitioner (NP) license in North Carolina
  • Board certified via AANP or ANCC
  • Valid CPR (BLS) certification
  • MSN degree from an accredited nursing program
  • At least six months of current NP experience preferred
