Job Description
Seeking an experienced Nurse Practitioner to deliver essential home health risk assessments for Medicare Advantage plan members residing in underserved communities. This per diem opportunity offers flexible scheduling and the ability to tailor your hours based on personal availabilitywork as much or as little as you prefer, with shifts available any day of the week.
Schedule & Hours:
- Choose your own shifts, Monday through Sunday
- Minimum weekly or bi-weekly hourly commitment
- Assignments may be in various locations within Guilford County, including Summerfield, Oak Ridge, Stokesdale, and surrounding neighborhoods
Key Responsibilities:
- Complete comprehensive in-home health risk assessments, typically lasting about 45 minutes each
- Evaluate patients prior and current medical history, review chronic and acute conditions, and assess medication usage
- Educate members regarding their medical conditions, preventive care, and medications
- Identify gaps in care and recommend interventions to improve patient outcomes
- No requirement for medication prescribing or development of treatment plans
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certification (AANP or ANCC)
- MSN degree from an accredited nursing program
- Current BLS (CPR) certification
- At least 6 months of recent Nurse Practitioner experience preferred
