Job Description

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct in-home risk assessments, including annual evaluations and post-discharge visits, for members residing in Keokuk, Fort Madison, Montrose, and nearby locations
  • Complete comprehensive reviews of prior and current medical history, medical conditions, and medication usage
  • Educate individuals on their health status, conditions, and medication regimens to support informed healthcare decisions
  • Identify gaps in care and provide actionable recommendations for improvement
  • No responsibilities for developing treatment plans or prescribing medications

Desired Qualifications:

  • Active Nurse Practitioner license in Iowa
  • Master of Science in Nursing (MSN) from an accredited institution
  • Board Certification as a Nurse Practitioner (AANP or ANCC)
  • Current Basic Life Support (BLS/CPR) certification
  • At least 6 months of recent Nurse Practitioner experience preferred
