Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct in-home risk assessments, including annual evaluations and post-discharge visits, for members residing in Keokuk, Fort Madison, Montrose, and nearby locations
- Complete comprehensive reviews of prior and current medical history, medical conditions, and medication usage
- Educate individuals on their health status, conditions, and medication regimens to support informed healthcare decisions
- Identify gaps in care and provide actionable recommendations for improvement
- No responsibilities for developing treatment plans or prescribing medications
Desired Qualifications:
- Active Nurse Practitioner license in Iowa
- Master of Science in Nursing (MSN) from an accredited institution
- Board Certification as a Nurse Practitioner (AANP or ANCC)
- Current Basic Life Support (BLS/CPR) certification
- At least 6 months of recent Nurse Practitioner experience preferred
