Job Description
Serve as a vital resource for patients by conducting in-home health risk assessments for Medicare Advantage members throughout Portage County, OH. As a Nurse Practitioner specializing in home health, you will have the unique opportunity to deliver individualized care and gather crucial patient data during visits lasting approximately 45 minutes each. Shifts are scheduled flexibly, empowering you to create your own workweek with a minimum weekly or bi-weekly hourly commitmentideal for professionals seeking per diem opportunities that offer both autonomy and impact.

Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage health plan members
- Evaluate comprehensive medical history, current conditions, and medication use
- Educate patients on existing health conditions and medication regimens
- Identify and address gaps in care, making recommendations for resolution
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR (BLS) certification
- Board Certification via AANP or ANCC
- At least 6 months of current experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited nursing school
