Schedule:

• Flexible shifts available Monday through Sunday
• Freedom to create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete comprehensive home health risk assessments for Medicare Advantage members
• Review and document current and previous medical history, health conditions, and medication use
• Offer education to patients on medical conditions and medication management
• Identify and address gaps in care, making recommendations to support positive outcomes
• No requirement to prescribe medications or develop treatment plans

Qualifications:

• Active Nurse Practitioner license in Washington State
• Current CPR certification (BLS)
• Board Certification via AANP or ANCC
• 6 months of recent NP experience preferred
• Masters degree in Nursing from an accredited institution
