Job Description

Embark on a fulfilling per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. This role focuses on providing essential care to Medicare Advantage plan members in Wilson County, serving towns such as Wilson, Elm City, and Lucama.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Set your minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members
  • Review prior and current medical history, underlying conditions, and medication history for each patient
  • Educate members about their medical conditions and medications
  • Identify gaps in care and offer recommendations to improve overall health outcomes
  • All visits are assessment-focused: no treatment plan creation or medication prescribing required

Qualifications:

  • Current Nurse Practitioner license in North Carolina
  • Board certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited program
  • Current CPR certification (BLS)
  • At least 6 months of recent Nurse Practitioner experience preferred
