Experience the rewarding impact of providing patient-centered care as a Nurse Practitioner specializing in home health risk assessments. In this per diem opportunity, you will play a vital role in supporting Medicare Advantage members residing in Kellogg and surrounding areas within Shoshone County by conducting thorough and compassionate assessments in their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule for maximum autonomy
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate prior and current medical history, current medical conditions, and medication history
- Offer patient education tailored to medical conditions and prescribed medications
- Identify gaps in care and provide clear recommendations for care improvement
- Focus exclusively on assessmentno treatment plan creation or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Idaho
- Board Certification through AANP or ANCC
- Current and valid CPR certification (BLS)
- MSN degree from an accredited institution
- At least 6 months of NP experience preferred
