Job Description

Experience the rewarding impact of providing patient-centered care as a Nurse Practitioner specializing in home health risk assessments. In this per diem opportunity, you will play a vital role in supporting Medicare Advantage members residing in Kellogg and surrounding areas within Shoshone County by conducting thorough and compassionate assessments in their homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule for maximum autonomy
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate prior and current medical history, current medical conditions, and medication history
  • Offer patient education tailored to medical conditions and prescribed medications
  • Identify gaps in care and provide clear recommendations for care improvement
  • Focus exclusively on assessmentno treatment plan creation or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Board Certification through AANP or ANCC
  • Current and valid CPR certification (BLS)
  • MSN degree from an accredited institution
  • At least 6 months of NP experience preferred
