Nurse Practitioners with a passion for home health are invited to support Medicare Advantage members by conducting comprehensive risk assessments in Shoshone County, including Kellogg, Pinehurst, and Osburn. This flexible, per diem opportunity allows you to make a meaningful impact through personalized, in-home evaluations.
Schedule
- Flexible shifts available Monday through Sunday
- Self-schedulingdesign your own workweek
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities
- Perform thorough annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate prior/current medical history and ongoing medical conditions
- Assess medication history and provide clear education on medical conditions and medication management
- Identify care gaps; offer recommendations to enhance care quality
- Deliver post-discharge visits as needed
- No requirement to develop treatment plans or prescribe medications
Qualifications
- Active Nurse Practitioner license in Idaho
- Board Certification (AANP or ANCC)
- Current CPR certification (BLS)
- At least 6 months of recent NP experience preferred
- MSN from an accredited nursing school
