Step into a dynamic Nurse Practitioner opportunity focused on home health risk assessments with flexible, per diem scheduling. This role entails visiting Medicare Advantage plan members within Kalamazoo County, conducting thorough assessments that inform patient care and health plan management.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own schedule to fit your needs
- Minimum commitment of weekly or bi-weekly hours, as desired
Key Responsibilities:
- Perform comprehensive annual risk assessments during home visits (each averaging 45 minutes)
- Evaluate medical history, current health conditions, and medication regimens for assigned patients
- Deliver clear, relevant education regarding medical conditions and prescribed medications
- Identify and document any gaps in patient care, providing actionable recommendations to resolve them
- Visit locations throughout Kalamazoo County, which may include Kalamazoo, Portage, and Oshtemo during your shifts
- No treatment plan development or medication prescribing is required
Qualifications:
- Active Nurse Practitioner license in Michigan
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- Minimum of six months recent experience as a Nurse Practitioner is preferred
For each evaluation completed, compensation ranges from $80-$90, accompanied by a mileage stipend and allocated administrative pay to support your time and effort.
