Step into a critical role by supporting the well-being of Medicare Advantage members through comprehensive home health risk assessments across Kalamazoo County. As a Nurse Practitioner focusing on home-based care, you'll provide valuable evaluations and health education for patients in locations such as Kalamazoo, Portage, and Oshtemo. This per diem opportunity allows you to make a meaningful impact, helping bridge gaps in careall while enjoying a flexible schedule that adapts to your lifestyle.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Options to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct home health risk assessments for Medicare Advantage members, averaging approximately 45 minutes per assessment
  • Evaluate patients medical history, current medical conditions, and medication usage
  • Deliver patient education regarding their health conditions and prescribed medications
  • Identify gaps in care, offering actionable recommendations for improved outcomes
  • Visits may include annual assessments or post-discharge follow-ups
  • Prescribing medication or creating treatment plans is not required

Required Qualifications:

  • Active Nurse Practitioner license in Michigan
  • Current BLS/CPR certification
  • Board Certified through AANP or ANCC
  • Minimum of six months recent Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
