Elevate your practice as a Nurse Practitioner specializing in home health risk assessments on a per diem basis. You'll enjoy exceptional flexibility, making a meaningful impact while gaining firsthand insight into members' needs throughout Lane County.

Schedule/Hours:

• Choose your own schedule with flexible shifts offered Monday through Sunday
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform thorough home health risk assessments for Medicare Advantage members
• Review each patients medical, surgical, and medication history
• Deliver patient education regarding chronic conditions, disease management, and medication adherence
• Identify any care gaps and offer evidence-based recommendations for resolution
• No requirement for developing treatment plans or prescribing medications

You'll travel within Lane County, evaluating members residing in Eugene, Springfield, Elmira, Cottage Grove, and surrounding areas. Each assessment lasts approximately 45 minutes and may involve annual evaluations or post-discharge follow-ups.

Qualifications:

• Current, unrestricted Nurse Practitioner license in Oregon
• Active CPR certification (BLS)
• National Board Certification through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• At least 6 months of recent experience as a Nurse Practitioner preferred
