This per diem opportunity focuses on home health risk assessments for Medicare Advantage plan members within Latah County, providing valuable evaluations in individual homes across communities such as Moscow, Genesee, and Troy.
Schedule:
- Flexible shifts available between Monday and Sunday

- Minimum weekly or bi-weekly hourly commitments for optimal work-life balance
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
- Review and evaluate patients medical histories, conditions, and medication lists
- Educate patients about their medical conditions and medications to enhance health literacy
- Identify care gaps and deliver tailored recommendations for improved outcomes
- Role does not require prescribing medications or developing treatment plans
Qualifications:
- Active Nurse Practitioner license in Idaho
- Board Certification via AANP or ANCC
- Current CPR certification (BLS)
- Minimum of 6 months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
