Job Description

Seeking a dynamic and independent Nurse Practitioner with home health experience to provide risk assessments in a per diem capacity. This essential role centers on conducting in-home evaluations for Medicare Advantage plan members throughout Nez Perce County, including areas such as Lewiston, Lapwai, and Peck. Each member visit generally lasts about 45 minutes and may cover either annual health assessments or post-discharge check-ins. Flexible scheduling is available, with shifts offered between Monday and Sunday, allowing you to create your own schedule and fulfill a minimum weekly or bi-weekly hourly commitment.

Key Responsibilities:

  • Perform comprehensive annual health risk assessments in patients homes
  • Review current and prior medical history, ongoing conditions, and medications
  • Educate patients on their health status and prescribed medications
  • Pinpoint gaps in care and offer recommendations to address these
  • No development of treatment plans or prescription writing needed

Qualifications:

  • Active Idaho Nurse Practitioner license
  • Board Certified via AANP or ANCC
  • Current CPR certification (BLS required)
  • MSN from an accredited nursing program
  • Minimum of 6 months experience as a Nurse Practitioner preferred
  • Experience with home health assessments is a plus

For each evaluation, compensation ranges from $80 to $90, with additional mileage stipend and paid administrative time.
