As a Nurse Practitioner specializing in home health, youll perform in-depth risk assessments for Medicare Advantage members located throughout Woodruff County, including Augusta, McCrory, and Cotton Plant. This rewarding role involves independent visits to patient homes, allowing you to make a meaningful impact on individuals lives while enjoying autonomy over your schedule.
Schedule:
- Enjoy flexible shifts, Monday through Sunday
- Design your own work schedule
- Choose a minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Conduct home health risk assessments, typically lasting 45 minutes, for Medicare Advantage plan members in Woodruff County
- Perform comprehensive evaluations, including review of current and prior medical history, medical conditions, and medication lists
- Educate individuals about their medical conditions and medications during visits
- Identify care gaps and recommend next steps for overall wellness
- Focus solely on assessment and educationno need to prescribe medications or create treatment plans
Desired Experience & Credentials:
- Licensed Nurse Practitioner in the state of Arkansas
- Board certification through AANP or ANCC
- Masters Degree in Nursing (MSN) from an accredited institution
- Current Basic Life Support (BLS/CPR) certification
- At least six months of Nurse Practitioner experience preferred
