Enjoy a dynamic per diem opportunity as a Nurse Practitioner focused on home health risk assessments for Medicare Advantage plan members in Jasper County, MO. In this role, you'll visit patients in their residences throughout Joplin, Carthage, Webb City, and Carterville, ensuring they receive personalized, comprehensive health evaluations. Each visit typically takes about 45 minutes and may involve annual health assessments or post-discharge evaluations. This flexible position allows you to craft your own schedule, with shifts available every day of the week and the option to set your preferred hourly commitment on a weekly or bi-weekly basis.

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members in their homes
• Assess prior/current medical and medication histories along with medical conditions
• Educate patients about their medical issues and medications
• Spot gaps in care and recommend solutions
• No prescribing or treatment planning is required

Qualifications:

• Active Nurse Practitioner license in Missouri
• Certified in Basic Life Support (BLS/CPR)
• Board certified through AANP or ANCC
• At least 6 months of current Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited school

This per diem role provides competitive compensation of $80 to $90 for each evaluation, along with mileage and administrative time stipends.
