Make a meaningful difference in community health by providing home-based risk assessments to Medicare Advantage members throughout Fulton County, NY. This per diem position is an excellent opportunity for Nurse Practitioners interested in flexible scheduling and a patient-centered approach outside of a traditional clinic setting.
Schedule:
- Flexible shifts available Monday through Sunday
- You have the ability to create your own schedule
- Select your minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual health risk assessments for Medicare Advantage plan members in home settings
- Evaluate medical histories, current health conditions, and medication profiles
- Educate patients about their medical conditions and prescribed medications
- Spot gaps in care, offering recommendations and resources to enhance patient outcomes
- No requirement to create treatment plans or prescribe medications, allowing you to focus on thorough assessments
Desired Qualifications:
- Active Nurse Practitioner license in New York
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
