Role Overview: In this role, you will complete home health risk assessments for Medicare Advantage members in communities such as Gloversville, Johnstown, and Broadalbin. Assessments will generally take about 45 minutes and may include annual health evaluations or post-discharge visits, with no requirement to develop treatment plans or prescribe medications.
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members
- Evaluate prior/current medical history, chronic conditions, and medication use
- Educate members regarding their medical conditions and medication regimens
- Identify care gaps and offer recommendations to improve patient outcomes
- Complete all documentation and assessments accurately within the set timelines
Required Qualifications:
- Active Nurse Practitioner license in NY
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- Master of Science in Nursing (MSN) from an accredited program
- Preferred: At least 6 months of NP experience (home health experience valued)
