Advance your career as a Nurse Practitioner specializing in home health by conducting in-depth risk assessments for Medicare Advantage plan members throughout Broome County, NY. Youll engage with patients in their homes, ensuring quality health assessments and supporting their ongoing care needs. This per diem opportunity offers flexibility and autonomyperfect for experienced nurse practitioners seeking meaningful, patient-centered work with control over their schedule.
Schedule:
- Flexible shifts available seven days a week
- Design your own schedule to match personal commitments
- Choose weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate prior and current medical histories, existing conditions, and medication use
- Educate members on chronic diseases, preventive strategies, and medication protocols
- Detect gaps in current care management and provide actionable recommendations to improve patient outcomes
- Please note: This role does not involve developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in New York State
- Board certification through AANP or ANCC
- Current CPR (BLS) certification
- Master of Science in Nursing (MSN) from an accredited institution
- Minimum of six months experience as a practicing NP preferred
As part of this dedicated healthcare team, youll receive compensation of $80$90 for each completed evaluation, along with a mileage stipend and additional administrative time considered.
