Schedule:

  • Flexible shifts available seven days a week
  • Design your own schedule to match personal commitments
  • Choose weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical histories, existing conditions, and medication use
  • Educate members on chronic diseases, preventive strategies, and medication protocols
  • Detect gaps in current care management and provide actionable recommendations to improve patient outcomes
  • Please note: This role does not involve developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in New York State
  • Board certification through AANP or ANCC
  • Current CPR (BLS) certification
  • Master of Science in Nursing (MSN) from an accredited institution
  • Minimum of six months experience as a practicing NP preferred

As part of this dedicated healthcare team, youll receive compensation of $80$90 for each completed evaluation, along with a mileage stipend and additional administrative time considered.
