Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform thorough annual home health risk assessments for Medicare Advantage health plan members in their residences
- Assess both prior and current medical history, health conditions, and comprehensive medication profiles
- Deliver patient-centered education on conditions, medications, and preventive care
- Pinpoint care gaps and recommend actionable resolutions to elevate patient outcomes
- Focus strictly on evaluation and educationno prescribing or treatment plan development necessary
Qualifications:
- Active and unrestricted Nurse Practitioner license in New York
- Certified in Basic Life Support (BLS/CPR)
- Board certification from AANP or ANCC
- Six months or more of recent NP experience preferred
- Master of Science in Nursing (MSN) degree from an accredited institution
Your work will predominantly involve providing critical assessments, empowering patients in the greater Broome County area, including communities like Binghamton, Vestal, Union, and Johnson City.
This position offers $80-$90 per completed evaluation, with additional compensation for mileage and administrative time.
