As a Nurse Practitioner specializing in home health, you will provide vital risk assessments for Medicare Advantage plan members residing in communities such as Teague, Fairfield, Wortham, and Kirvin. Each assessment visit typically takes about 45 minutes and supports overall patient well-being, from annual evaluations to post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit personal and professional commitments
- Choose a minimum commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Perform comprehensive at-home risk assessments for Medicare Advantage participants
- Review and evaluate previous and current medical history, conditions, and medication regimens
- Offer patient-focused education on diagnosed health concerns and medications
- Identify gaps in care and recommend appropriate steps to address them
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Texas
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Current CPR (BLS) certification
- Preferably six months of recent Nurse Practitioner experience, particularly in home health
