Advance your practice in a dynamic per diem opportunity providing home health risk assessments within the community. This role focuses on improving outcomes for Medicare Advantage members, supporting their health journey through comprehensive evaluations. Youll visit individuals in their residences to perform structured, scheduled risk assessmentseach typically lasting 45 minuteswith location assignments possible throughout Union County.
Schedule:
- Flexible shifts available Monday through Sunday

Responsibilities:
- Conduct detailed annual health risk assessments on Medicare Advantage members
- Review members medical history, current conditions, and medication usage
- Educate patients on their conditions, medication use, and preventive care
- Identify and address gaps in care; offer practical recommendations
- Focus solely on assessmentno treatment planning or medication prescribing required
Qualifications:
- Valid Nurse Practitioner license in New Jersey
- Board certified by AANP or ANCC
- Current BLS (CPR) certification
- MSN from an accredited nursing program
- At least 6 months of current nurse practitioner experience preferred
