This per diem role offers remarkable flexibility with self-scheduling options throughout the week, including evening and weekend shifts. Coverage areas include Jersey City and other locations in Hudson County, such as Bayonne and Union City, with each assessment averaging 45 minutes.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Design your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Core Responsibilities:

  • Conduct comprehensive annual home health risk assessments in patients residences
  • Evaluate medical histories, current conditions, and medication usage
  • Educate plan members about their medical conditions, treatment adherence, and medication management
  • Identify gaps in care and recommend actions or follow-up to address these needs
  • No treatment planning or prescribingfocus exclusively on assessment, education, and care recommendation

Qualifications:

  • Valid Nurse Practitioner license in New Jersey
  • Board Certification (AANP or ANCC)
  • Current CPR (BLS) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience preferred, especially in home health
