Advance your career as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members. This per diem role offers the opportunity to work autonomously, making a significant impact on patient care across Union County, NJ, including areas such as Elizabeth, Plainfield, and Linden.
Schedule:
- Flexible shifts available any day of the week (Monday through Sunday)
- Self-scheduling enables you to create your own work hours
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive home health risk assessments, each typically lasting 45 minutes
- Assess and document prior/current medical history, health conditions, and medication regimen
- Educate patients about health conditions, preventive care, and medication usage
- Identify care gaps and recommend further steps to address health concerns
- No prescribing of medications or development of treatment plans is involved
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Board Certification via AANP or ANCC
- Current BLS CPR certification
- MSN from an accredited nursing program
- Minimum six months recent nurse practitioner experience preferred
This position provides competitive earnings of $80-$90 per evaluation, as well as a mileage stipend and administrative compensation.
