Experienced Nurse Practitioners are needed for per diem home health risk assessments serving Medicare Advantage members throughout Morris County, NJ, including areas such as Parsippany-Troy Hills, Mount Olive, and Randolph. This opportunity allows clinicians to conduct comprehensive evaluations in the members home, focusing on annual health reviews and post-discharge visits.

Schedule:

• Flexible shifts available seven days a week
• Create your own schedule to suit your lifestyle
• Choose a minimum hourly commitment on a weekly or bi-weekly basis

Primary Responsibilities:

• Perform detailed home-based risk assessments on Medicare Advantage plan members
• Review and document prior/current medical history, conditions, and medication lists
• Educate members about their health conditions and medications
• Identify care gaps and recommend resolutions as needed
• No requirement for treatment plan creation or medication prescribing

Desired Qualifications:

• Active Nurse Practitioner license in the state of New Jersey
• Board Certification through AANP or ANCC
• Current CPR/BLS certification
• MSN from an accredited nursing program
• At least 6 months of recent NP experience (preferred)
