Enjoy the autonomy and flexibility of providing high-quality patient care as a Nurse Practitioner specializing in home health within dynamic communities. This per diem role offers a unique opportunity to conduct home-based risk assessments for Medicare Advantage plan members residing in various areas of Morris County, NJ, including Parsippany-Troy Hills, Mount Olive, and Randolph.
Schedule:
- Flexible shifts available between Monday and Sunday
- Freedom to create your own schedule
- Minimum hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Complete thorough annual home health risk assessments for Medicare Advantage health plan members
- Review and evaluate patients prior and current medical histories, conditions, and medication usage
- Provide patient education related to diagnoses, management, and medications
- Identify potential gaps in care and suggest appropriate recommendations
- Visits may include annual wellness assessments or post-discharge follow-ups
- No development of treatment plans or medication prescribing required
Qualifications:
- Active Nurse Practitioner License in New Jersey
- Current CPR (BLS) certification
- Board Certification via AANP or ANCC required
- MSN degree from an accredited school of nursing
- Minimum of 6 months of recent NP experience preferred
You will be compensated between $80 and $90 per evaluation, plus receive a mileage stipend and compensation for administrative time.
