This per diem opportunity offers maximum flexibilitychoose your own schedule and set your own weekly or bi-weekly hourly commitments. Enjoy diverse work across areas including Jersey City, Bayonne, and Union City, with visits averaging 45 minutes and focused on patient evaluation and care guidance.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients prior and current medical history, conditions, and medications
- Provide clear education to patients about their health status and medication regimen
- Identify gaps in care and offer actionable recommendations to address them
- Assessments do not require development of treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Current CPR certification (BLS required)
- National board certification through AANP or ANCC
- MSN degree from an accredited nursing school
- At least six months of Nurse Practitioner experience preferred
