Experienced Nurse Practitioners seeking flexible, per diem opportunities will find this role rewarding and impactful. This position involves conducting home health risk assessments for Medicare Advantage plan members across various locations within Jerome County, including Jerome, Hazelton, and Eden. Your clinical expertise will help improve health outcomes for members in their own homes, focusing on annual assessments or post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive annual health risk assessments for Medicare Advantage members
- Assess medical history, current conditions, and medication profiles
- Deliver patient education related to diagnoses and medications
- Identify gaps in care and make recommendations for follow-up as needed
- No requirement to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in Idaho
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of recent NP experience preferred
