Advance your practice and provide high-impact care as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers flexible scheduling within Jerome County, ID, covering areas such as Jerome, Hazelton, and Eden. As a valued healthcare professional, youll conduct in-home risk assessments supporting Medicare Advantage members, assisting with preventive care, chronic disease management, and comprehensive wellness evaluations.
Schedule:
- Flexible shifts available Monday through Sunday

- Choose minimum weekly or bi-weekly hour commitments to fit your needs
Key Responsibilities:
- Perform thorough annual home health risk assessments for Medicare Advantage members
- Review and evaluate each patients medical history, current conditions, and medication use
- Deliver education regarding medical conditions and medications to patients
- Proactively identify care gaps and recommend solutions to address them
- Focus strictly on assessmentsno prescribing or treatment plan development required
Qualifications:
- Valid Nurse Practitioner license in the state of Idaho (ID)
- Current BLS certification (CPR)
- Board certified through AANP or ANCC
- Minimum six months of recent NP experience preferred
- MSN degree from an accredited nursing school
For each completed evaluation, compensation ranges from $80-$90, with additional stipends for mileage and administrative time.
