In this role, you will make a meaningful impact by completing comprehensive in-home assessments of Medicare Advantage members, helping to bridge care gaps and support each patients well-being in the comfort of their own residence.

Key Responsibilities:

• Conduct in-home annual risk assessments for Medicare Advantage members
• Assess current and prior medical history, ongoing conditions, and medication usage
• Provide education to members regarding their medical conditions and medication regimens
• Identify gaps in care and deliver actionable recommendations to promote patient health
• No responsibilities related to treatment planning or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Ohio
• Board Certification through AANP or ANCC
• Current BLS (CPR) certification
• MSN degree from an accredited nursing program
• At least six months of recent Nurse Practitioner experience preferred

Compensation for this position is highly competitive, with $80$90 provided for each evaluation completed, plus a mileage stipend and compensation for administrative time.

This position offers the flexibility to serve a diverse population in Ashtabula County, including areas such as Ashtabula, Conneaut, and Geneva. Schedule assignments may vary each shift, providing a rewarding and varied work environment.
