Job Description

This per diem opportunity is perfect for energetic Nurse Practitioners with home health experience, looking for a flexible schedule while serving Medicare Advantage plan members in Cocke County, TN, including areas such as Newport and Parrottsville.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to fit your commitments
  • Opportunity for weekly or bi-weekly shift commitments

Key Responsibilities:

  • Conduct comprehensive home health risk assessments, typically lasting 45 minutes per patient
  • Review and evaluate previous and current medical histories, existing conditions, and medication usage
  • Provide thorough patient education regarding medical conditions and medication management
  • Identify and communicate any gaps in care, offering recommendations for improved outcomes
  • No requirements for developing treatment plans or prescribing medications

Qualifications:

  • Unencumbered Nurse Practitioner license in the state of Tennessee
  • Active Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • Current CPR (BLS) certification
  • At least six months of recent NP experience (preferred)
  • Previous home health experience is a plus
