This per diem opportunity is perfect for energetic Nurse Practitioners with home health experience, looking for a flexible schedule while serving Medicare Advantage plan members in Cocke County, TN, including areas such as Newport and Parrottsville.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your commitments
- Opportunity for weekly or bi-weekly shift commitments
Key Responsibilities:
- Conduct comprehensive home health risk assessments, typically lasting 45 minutes per patient
- Review and evaluate previous and current medical histories, existing conditions, and medication usage
- Provide thorough patient education regarding medical conditions and medication management
- Identify and communicate any gaps in care, offering recommendations for improved outcomes
- No requirements for developing treatment plans or prescribing medications
Qualifications:
- Unencumbered Nurse Practitioner license in the state of Tennessee
- Active Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Current CPR (BLS) certification
- At least six months of recent NP experience (preferred)
- Previous home health experience is a plus
