Job Description
Seeking an experienced Nurse Practitioner to perform home health risk assessments on a per diem basis. The role involves conducting comprehensive in-home evaluations with Medicare Advantage plan members across Cocke County, including Newport and Parrottsville. Each assessment typically takes around 45 minutes and may consist of annual health checks or post-discharge follow-ups. Flexibility is built into this opportunity, with the ability to select shifts that best fit within your schedule.
Schedule:
- Flexible shifts available any day of the week (MondaySunday)
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Assess current and prior medical histories, health conditions, and medication usage
- Offer education regarding health conditions and appropriate medication usage
- Identify gaps in care and suggest recommendations for improvement
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Current Nurse Practitioner license for Tennessee
- CPR certification (BLS)
- Board certified by AANP or ANCC
- Minimum of six months experience as a nurse practitioner preferred
- Master of Science in Nursing (MSN) from an accredited institution
