Take on a rewarding per diem opportunity as a Nurse Practitioner focused on home health risk assessments throughout Boone County. In this dynamic role, you will visit Medicare Advantage plan members in their homesserving communities such as Harrison, Diamond City, and Bergmanto deliver high-quality and compassionate care. Each individualized risk assessment takes about 45 minutes and may involve annual health evaluations or post-discharge visits tailored to the needs of each member.

- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage members
- Assess and review past and current medical history, ongoing conditions, and all medications
- Deliver patient education regarding health conditions and medication management
- Identify and report care gaps, making appropriate recommendations
- Focus exclusively on evaluationno treatment planning or prescribing required
Qualifications:
- Active and unencumbered Nurse Practitioner license in Arkansas
- Current BLS (CPR) certification
- Board Certification via AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
