Take on an impactful role conducting home health risk assessments within Boone County, Arkansas, evaluating and supporting the well-being of Medicare Advantage members in cities such as Harrison, Diamond City, and Bergman. This per diem role is designed for a dedicated Nurse Practitioner seeking variety, autonomy, and the opportunity to make a meaningful difference through individualized assessments and education in patients homes.
Schedule:

Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
- Gather detailed medical histories, reviewing previous and current conditions as well as medication usage
- Educate patients on their medical conditions and prescribed medications
- Identify gaps in care or potential health risks, providing relevant recommendations
- Focus exclusively on evaluationno treatment plan creation or prescribing required
Qualifications:
- Active Nurse Practitioner License in Arkansas
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of recent Nurse Practitioner experience preferred
