Job Description
Deliver high-quality care by performing in-depth home health risk assessments for Medicare Advantage members throughout Greene County. This per diem opportunity offers Nurse Practitioners the flexibility to set their own schedules, allowing for an ideal work-life balance while making a direct impact on patient health outcomes. Assessments typically take about 45 minutes per patient and may vary, including both annual evaluations and post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to suit your preferences
- Commit to a manageable weekly or bi-weekly hourly minimum
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review current and prior medical history, conditions, and medications
- Educate members regarding their diagnoses and prescribed medications
- Identify gaps in care and recommend actions to address them
- Visits do not require developing treatment plans or prescribing medications
Qualifications & Desired Experience:
- Active Nurse Practitioner license in Indiana
- Current BLS (CPR) certification
- Board certification through the AANP or ANCC
- Minimum of six months of experience as an NP (preferred)
- MSN degree from an accredited nursing program
In this role, you will be compensated $80-$90 per completed evaluation, with additional mileage stipend and compensation for administrative time. Work sites are located within Stockton Township and other areas of Greene County, including Linton, Bloomfield, and Jasonville.
