Nurse Practitioners passionate about providing comprehensive, patient-focused care are needed for per diem home health risk assessments across Greene County, IN. This role offers exceptional flexibility, allowing you to design a schedule that fits your lifestyle while making a meaningful impact in Jasonville, Linton, Bloomfield, Stockton Township, and nearby communities.
Schedule:
- Flexible shifts available seven days a week
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual in-home health risk assessments for Medicare Advantage plan members
- Carefully evaluate patients medical histories, chronic conditions, and medication regimens
- Deliver clear education to patients regarding their health conditions and medications
- Identify gaps in care and recommend appropriate next steps
- Visits focus on assessmentsno development of treatment plans or medication prescribing required
Qualifications:
- Active and unrestricted Nurse Practitioner license in Indiana
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- Minimum of 6 months recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
This per diem opportunity compensates you at $80-$90 per completed evaluation, along with mileage reimbursement and paid administrative time.
