Advance healthcare from the comfort of patient homes by providing comprehensive risk assessments in Washington County, NC. This per diem opportunity is ideal for a Nurse Practitioner seeking autonomy, professional impact, and schedule flexibility. Home health visits will vary, primarily comprising annual health assessments or post-discharge evaluations of Medicare Advantage plan members within the communities of Plymouth, Roper, Creswell, and other Washington County locations.
Schedule:
- Flexible shifts available seven days a week
- Design your own schedule to fit personal and professional needs
- Commit to minimum weekly or bi-weekly hours
Core Responsibilities:
- Conduct home health risk assessments, typically 45 minutes per visit
- Review and evaluate prior/current medical histories, conditions, and medication regimens
- Provide patient education regarding their conditions and medications
- Identify gaps in care and recommend appropriate next steps
- No requirement to develop treatment plans or prescribe medications
Desired Qualifications:
- Active and unrestricted Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- MSN from an accredited nursing program
- Minimum 6 months of NP experience preferred
