Discover a rewarding opportunity as a Nurse Practitioner specializing in home health risk assessments. This per diem position offers flexibility and the chance to make a meaningful impact on Medicare Advantage plan members across Washington County. Assignments may include locations such as Plymouth, Roper, and Creswell, with home visits scheduled independently within your preferred availability.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule based on availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Assess and document prior/current medical history, conditions, and medication regimens
- Offer patient education regarding conditions, treatments, and medications
- Identify gaps in care, provide informed recommendations for closing those gaps
- No involvement in treatment planning or medication prescribing
Qualifications:
- Valid Nurse Practitioner license in North Carolina
- Current BLS (CPR) certification
- Board certification through AANP or ANCC
- MSN from an accredited nursing program
- At least 6 months of recent NP experience (preferred)
