Seeking an experienced Nurse Practitioner to conduct home health risk assessments for Medicare Advantage plan members across Fentress County, TN, including Jamestown, Allardt, and Grimsley. This per diem opportunity allows you to make a significant impact in preventive healthcare while maintaining a flexible work schedule that fits your lifestyle.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage health plan members in their residences
- Assess prior and current medical history, conditions, and medication usage
- Educate patients on their medical conditions and proper medication management
- Identify gaps in care and suggest actionable recommendations for improvement
- Complete thorough documentation; no treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months recent NP experience, preferably in home health or related setting
- Master of Science in Nursing (MSN) from an accredited nursing program
