Bring your expertise as a Nurse Practitioner to a dynamic per diem opportunity focused on providing essential care through home health risk assessments. This position serves Medicare Advantage plan members living throughout Fentress County, including the areas of Jamestown, Allardt, and Grimsley.
Schedule:
- Flexible shifts available between Monday and Sunday
- Design your own work schedule
- Committed hours can be set weekly or bi-weekly
Key Responsibilities:
- Conduct thorough annual home health risk assessments in patients homes
- Review and assess prior/current medical histories, ongoing medical conditions, and medication profiles
- Educate patients about their medical conditions and medication management
- Identify and communicate care gaps with recommendations for follow-up
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
This role offers robust compensation: earn $80$90 per evaluation, with additional stipends provided for mileage and administrative timecompensating you for both patient care and essential support activities.
